Payment
Payment may be made by cash, check and IvyPay. Although insurance is not accepted, I can provide a Superbill to PPO insurance for reimbursement. Please contact me if you have any questions regarding Superbill or session fees.
Cancellation Policy
A weekly appointment time is reserved specifically for you. Cancellations will require 24 hour notice to your weekly session time. If that prior notice is not received, you will be responsible for the full session fee.
Good Faith Estimate: Under Section 2799B-6 of the Public Health Service Act, health care providers are required to inform individuals who are not enrolled in a plan or coverage or federal healthcare program, or not seeking to file a claim with their plan or coverage orally and or in writing, Upon Request or at time of scheduling health care items and services to receive a “Good Faith Estimate” of expected charges. You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under law, health care providers need to give patients without insurance or who are not using insurance an estimate of the bill for medical services provided.
You can request a good faith estimate from an health care provider before initial services are provided. Make sure this statement is provided 1 day prior to initial visit. If you receive a bill at least $400 over the Good Faith Estimate, you will be able to dispute this bill. Keep a copy or picture of your Good Faith Estimate.
